GUIDELINE DETAILS
- Patient portion of the bill is due the day of service.
- There is a $50 reinstatement fee if your plan lapses.
- Cannot be used in conjunction with other dental plans.
- Care credit may not be used to pay Club premium.
- Non-refundable and/or non-transferable.
- Excludes complex dental work.

This program is a discount plan, not dental insurance. The Dental Care Club annual premium must be paid with either cash, check, or credit card.
Dental care club exclusions & limitations
- For treatment, in the sole opinion of our doctors, lies outside their scope.
- For referrals to specialists or specialized services.
- For hospitalization or hospital charges of any kind.
- For cost of dental care which are covered under automobile medical.
- For services or injuries covered under workers’ compensation.
